Transition Care

The Heidelberg University Hospital transition care team creates a "bridge" between hospital and the home environment. It helps tumor patients during their transition from hospital back into their own home and makes sure that patients receive optimal home care after their hospitalization.

The transition care team consists of certified nurses with advanced specialized training and many years of work experience. They possess solid and comprehensive specialized medical and nursing expertise.

The following question examples as well as further questions concerning nursing care may be addressed to the transition care team:

  1. Do I need assistance (for example, in personal hygiene) from an outpatient nursing service/welfare center and if so who will organize it?
  2. When can I or must I apply for a care allowance and who will help me do this?
  3. Do I need healthcare equipment at home (for example, homecare bed, walker, commode, oxygen apparatus...) and how will these be ordered and financed?
  4. Who will help me if I suffer from pain, have nutrition problems, wound care problems...?
  5. What happens if my condition deteriorates at home and I need more help? Is the transition care team my point of contact then, too?
  6. To whom can my family members address their questions?
